is for those NEW students entering First Grade in the Fall of 2023 and any NEW students
entering our program for the first time.

es un nuevo estudiante en nuestro programa por primera vez.

Form's Instructions		REGISTRATION INSTRUCTIONS
		Sacramental preparation is a two-year process. Once you submit this form, our office will be in touch with you to approve your child's registration and more information.

		FAMILY OPTIONS:
		Click "New Family" if you are not registered with our parish.
		Click "Update Existing Family" if you are registered with our parish. Enter your Parish "ID/Env." number.

		PROOF OF BAPTISM: If your child was not baptized at our parish, you must separately submit a copy of your child's baptismal certificate. (If your child is not yet baptized, we will need a copy of his/her birth certifcate). Please email it to: faithformation@sjecm.org (after you submit this registration form).

		REGISTRATION PAYMENT: Our registration payment may be made online:

		La preparacion sacramental es un proceso de dos anos. Una vez que usted envie este formulario, nuestra oficina se pondra en contacto con usted para aprobar el registro de su hijo/a y obtener mas informacion.

		REGISTRAR Y ACTUALIZAR:
		Presione aqui en "Nueva familia" si no esta registrado en nuestra parroquia.
		Presione aqui en "Actualizar familia existente" si esta registrado en nuestra parroquia. Ingrese su "ID / Env" numero de la parroquia.

		PRUEBA DE BAUTISMO
		Por favor llene el formulario a seguir en el enlace, para nuevos estudiantes. Si su hijo/a no fue bautizado en nuestra parroquia, necesitamos una copia de la acta de bautizo enviado por correo electronico a: faithformation@sjecm.org
